Billing and Insurance
We accept most local and many national insurance plans. If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.

Preparing for your appointment
- Please show your current insurance card when you check in for each appointment, and let us know if there are changes to your address or telephone number.
- Please bring a list of all medications you are currently taking.
- The name and address of the referring physician.
- For new patients, please download and fill out The New Patient Intake Form found under Patient Forms at the top.
- If you had any imaging done (MRI, CT, X-rays, etc.), please bring those studies to clinic with you.
Emergencies
For medical emergencies, call 911. You may also call the office number after hours to contact the physician on call for urgent questions.
Prescriptions
Prescription refills are available Monday through Friday 9:00 am - 5:00 pm. It is a good idea to order medications two days before you run out. Our staffdoes not refill prescriptions on weekends or during evening hours. We are not a pain clinic and DO NOT prescribe pain medications if we have not performed surgery on you.
Privacy
The Health Insurance Portability and Accountability Act (HIPAA) requires that individuals being treated for a medical condition provide written consent to a covered entity to discuss treatment for a medical condition with other individuals. A covered entity (generally, health care clearinghouses, employer sponsored health plans, health insurers, and medical service providers that engage in certain transactions) may disclose PHI (Protected Health Information) to facilitate treatment, payment, or health care operations without a patient's express written authorization. When there is a suspicion that the Privacy Rule has been violated, a complaint may be filed with the Department of Health and Human Services Office for Civil Rights (OCR).
